To download, use the mouse to right click either of the small boxes below.  A drop-down box will appear.  To print form choose "Open link in new window". When the new window opens, go to file and print. To save the form to your computer for future use, right click and choose "Save target as.." and save the file to the desktop.  After file has downloaded, go to desktop and double-click the file name.  The file will open in a new window.  Print the form(s) by choosing the printer icon at the top of the page in the new window.**
